A The following materials should be submitted:

1.  Application for measurement certification (in triplicate);
2. The table of measuring instruments of the quality inspection organization applying for certification (often referred to as \"capability table\");
3. Quality management manual;
4. Several copies of original records and test reports;
5. Self-examination form (self-examination is required for institutions that have not been pre-examined).
